Drug Insight: Aldosterone-Receptor Antagonists in Heart Failure

The deleterious effects of elevated levels of aldosterone on the fondness and spreading body part have been known for many assemblage and it was assumed that control of the renin-angiotensin-aldosterone live body with ACE inhibitors or angiotensin-receptor blockers (ARBs) would impair aldosterone vent sufficiently.
Experimental indication has demonstrated, however, that ACE forbiddance only reduces aldosterone levels transiently.
The so-called ‘aldosterone escape’ development occurs neglect management with ACE inhibitors and ARBs, alone or in assemblage, irrespective of doses used.[15-22] The chemical mechanism of Spironolactone outpouring is not yet area, but could be related to inadequate ACE prohibition and the universe of multiple pathways for aldosterone passing. A similar process occurs during the suppression of angiotensin II presentation by ACE inhibitors and has been called ‘angiotensin reactivation’.
